Terms and Conditions for Using Book A Doc, a Service Provided by Quando Business Solutions

Definitions

• "You," "Your," or "Customer" refers to any individual authorized to use our services for the purpose of medical or health-related appointment setting, contact facilitation, or referral concierge services.

 

Customer Responsibilities

By using our services, you agree to the following terms:

• Accuracy of Information: You confirm that the information you provide is true and accurate. Misrepresentation or false information may result in suspension or termination of your account.

• Referral Submission: You are either the named holder of the referral or have permission, due to a relationship, to submit the referral for the purpose of scheduling medical appointments on your behalf.

• No Modifications to Referrals: Referrals must be submitted as received, with no unauthorised alterations.

• Appointment Acceptance: You agree to accept the first available appointment within our health provider network. You are responsible for any cancellation fees imposed by the health provider or Book A Doc.

• Fees: See the attached fee schedule for applicable costs.

• Eligibility: You must be over 18 years old and a resident of Australia.

• Out-of-Pocket Expenses: You will be liable for any out-of-pocket or gap fees as stipulated by Book A Doc and will receive any applicable rebates accordingly.

• Contact Information: You must provide a valid mobile number and email address to receive appointment confirmations, billing details, and other relevant communications.

• Third-Party Authorization: You authorize Book A Doc and Quando Business Solutions to liaise with third parties, including but not limited to DVA, Medicare, health practitioners, and Workcover/Worksafe/Insurer , for the purpose of arranging medical appointments and processing payments. This authorisation gives consent to Bookadoc approval and consent to receive reports relating to the appointment arranged by Bookadoc.

• Credit Card Requirement: A valid credit card is required for the payment of relevant fees.

• Phone Communication: You consent to receive calls from the number 07-3473 5108 regarding your medical appointments.

• Patient Reports: The customer accepts that all reports will be sent directly to the referring practitioner and Bookadoc and will not be released directly to the patient, allowing you time to speak to the referring practitioner first.  Reports must be retrieved directly from the providers patient portal or referring practitioner.

 

Fee Schedule (Effective as of 1 October 2024)

*Workcover claims are exempt from the following fees if eligible*
 

All fees are quoted in Australian dollars (AUD)

• X-ray/OPG: $5 + 1.5% debit or credit card surcharge

• Ultrasound/CT: $8.50 + 1.5% debit or credit card surcharge

• MRI/PET/NUC MED/ECHO/Interventional: $10.00 + 1.5% debit or credit card surcharge

• Multiple Requests (e.g., X-ray and Ultrasound): $13.00 + 1.5% debit or credit card surcharge

• Cancelation fee if cancelled less than 24hours $50.00 + 1.5% debit or credit card surcharge

• Related fees may be billed directly to your health insurer.
  Surcharge refers to the bank fee for electronic fund transfers, as stipulated by our payment service provider.

 

Book A Doc Responsibilities

"We," "Us," "Book A Doc," or "Quando Business Solutions" refers to the service provider delivering the Book A Doc service. We commit to the following:

• Service Quality: We will deliver a high-standard, professional administrative concierge service to coordinate medical appointments, including diagnostic imaging and health rehabilitation services, on behalf of individuals and groups.

• Appointment Coordination: With your consent, we will manage all aspects of appointment preparation, post-appointment follow-ups, and report releases.

• Reports: We will ensure all relevant medical reports are received by fax as per your instructions.

• Cancellation Policy: No refunds will be provided for bookings canceled less than 24 hours prior to the appointment unless the cancellation has been properly communicated to Book A Doc.

• Fee Liability: Book A Doc is not liable for the payment of any medical examination fees. Unpaid invoices will incur an additional 10% charge. By submitting your request through www.bookadoc.net, you confirm that you are responsible for all fees related to the scheduled appointment and indemnify Book A Doc against any liabilities associated with the appointment.

• Refund Policy: We may provide a refund for booking fees at our discretion.

• Third-Party Invoicing: We will invoice third parties, such as insurers or Workcover, directly for relevant fees with your expressed consent.

• Service Channels: Our core service is online; however, telephone and online chat services may incur additional fees.

• Bookadoc/bookadoc.net is a service delivered by Quando Business and Management Solutions
